It is usually caused by coronary artery disease and is a precursor to myocardial infarction. Myocardial infarction is the result of prolonged myocardial ischemia, leading to irreversible myocardial cell damage or necrosis. This occurs when a coronary artery is completely occluded, usually by a blood clot, leading to the death of myocardial cells.
\nEtiology Coronary artery disease, atherosclerosis, vasospasm, hyperlipidemia, hypertension, smoking, diabetes, obesity, sedentary lifestyle, stress. Coronary artery disease, atherosclerosis, vasospasm, hyperlipidemia, hypertension, smoking, diabetes, obesity, sedentary lifestyle, stress.
\nAssessment Findings (including Diagnostics) Chest discomfort or pain, tightness, pressure, or squeezing, radiating to the neck, jaw, shoulders, arms, or back, shortness of breath, sweating, nausea, dizziness, fatigue, ECG changes, abnormal stress test. Elevated cardiac biomarkers, chest discomfort or pain, tightness, pressure, or squeezing, radiating to the neck, jaw, shoulders, arms, or back, shortness of breath, nausea, vomiting, diaphoresis, dysrhythmias, ECG changes, angiography.
\nNursing Diagnosis 1. Acute Pain related to myocardial ischemia.<\/p>\n
Anxiety related to fear of impending death or disability. 1. Decreased Cardiac Output related to myocardial cell damage\/necrosis.
\nAnxiety related to fear of impending death or disability.
\nInterventions (including meds and Surgical interventions) 1. Administer nitroglycerin to relieve chest pain.
\nAdminister beta-blockers to decrease myocardial oxygen demand.
\nAdminister antiplatelet or anticoagulant therapy to prevent thrombus formation. 1. Administer oxygen to increase myocardial oxygen supply.
\nAdminister nitroglycerin to relieve chest pain.
\nAdminister thrombolytic therapy to dissolve the blood clot.
\nAdminister antiplatelet or anticoagulant therapy to prevent further thrombus formation.
\nPrepare the patient for percutaneous coronary intervention (PCI) or coronary artery bypass graft (CABG) if indicated.
\nMonitor cardiac rhythm, blood pressure, oxygen saturation, and fluid and electrolyte balance.
\nProvide emotional support and education to the patient and family.<\/p>\n","protected":false},"excerpt":{"rendered":"
Complete the Cardiovascular Exemplar Table in its entirety.
