Assessment Overview

In this assessment, you will conduct a comprehensive analysis of community resources available to address a specific population health concern. You will identify a vulnerable population or community health issue, evaluate existing resources and services, identify gaps in care, and propose evidence-based recommendations to improve access and outcomes. This assessment emphasizes health promotion, disease prevention, and the role of community health nurses in advocating for underserved populations.

Assessment Instructions

Scenario: As a community health nurse, you have been asked to evaluate the resources available in your community (or a community of your choice) to address a specific population health concern. Your analysis will help identify strengths, gaps, and opportunities for improvement in community-based care delivery.

Assessment Requirements:

Part 1: Population Health Concern Identification (1-2 pages)

• Select and describe a specific population health concern (e.g., diabetes management, substance use disorder, maternal-child health, mental health access, food insecurity, homelessness, chronic disease management)
• Identify the vulnerable population most affected by this health concern
• Provide epidemiological data and statistics that demonstrate the significance of this issue in the selected community
• Discuss social determinants of health (SDOH) that contribute to health disparities for this population

Part 2: Community Resources Inventory (2-3 pages)

Identify and describe available resources in the following categories:

1. Healthcare Services:
   • Primary care clinics and federally qualified health centers (FQHCs)
   • Specialty care providers
   • Hospital and emergency services
   • Telehealth options
2. Public Health and Social Services:
   • County/state health departments
   • Nonprofit organizations and community-based programs
   • Food banks and nutrition assistance programs
   • Housing and shelter services
3. Support and Educational Services:
   • Support groups and peer counseling
   • Health education programs
   • Case management and care coordination services
   • Transportation assistance
4. Preventive and Screening Services:
   • Immunization clinics
   • Health screenings (cancer, cardiovascular, etc.)
   • Wellness programs
   • Disease prevention initiatives

Part 3: Gap Analysis and Barriers to Access (2-3 pages)

• Analyze gaps in available services and resources
• Identify barriers that prevent the vulnerable population from accessing existing resources (financial, geographic, cultural, linguistic, transportation, health literacy, stigma)
• Discuss how current resources address or fail to address social determinants of health
• Evaluate the cultural competence and accessibility of existing services

Part 4: Evidence-Based Recommendations (2-3 pages)

• Propose 3-5 specific, actionable recommendations to improve community resources and access to care
• Support each recommendation with current evidence-based research and best practices
• Describe the role of community health nurses in implementing these recommendations
• Discuss potential partnerships and collaborations (interprofessional teams, community organizations, government agencies)
• Address sustainability and long-term impact of proposed interventions

Part 5: Evaluation Plan (1 page)

• Identify measurable outcomes to evaluate the effectiveness of your recommendations
• Describe data collection methods and quality indicators
• Propose a timeline for implementation and evaluation

• Length: 8-11 pages (excluding title page and references)
• Format: APA 7th edition formatting
• References: Minimum of 5-7 current scholarly or professional sources (published within the last 5 years)
• Writing: Clear, professional, and well-organized with proper grammar and APA citations
• Supplemental Materials (Optional): You may include a table or chart summarizing community resources
• Submission: Upload as a Word document or PDF

Distinguished (4 points per criterion):

• Provides comprehensive analysis of population health concern with detailed epidemiological data and insightful discussion of social determinants
• Develops an exceptionally thorough inventory of community resources with specific contact information and service descriptions
• Conducts sophisticated gap analysis identifying multiple barriers with nuanced understanding of access issues
• Proposes innovative, evidence-based recommendations with clear implementation strategies and strong research support
• Presents detailed, measurable evaluation plan with specific metrics and realistic timeline

Proficient (3 points per criterion):

• Clearly identifies population health concern with relevant data and discussion of social determinants
• Provides thorough inventory of community resources across multiple categories
• Identifies significant gaps and barriers to access with appropriate analysis
• Proposes relevant evidence-based recommendations supported by current research
• Includes appropriate evaluation plan with measurable outcomes

Basic (2 points per criterion):

• Identifies population health concern but lacks depth in data or SDOH discussion
• Provides basic inventory of community resources but may be incomplete
• Mentions gaps and barriers but analysis lacks depth
• Proposes recommendations but evidence-based support is limited
• Includes basic evaluation strategies but lacks specificity

Non-Performance (0 points per criterion):

• Does not adequately identify a population health concern
• Fails to provide comprehensive resource inventory
• Does not identify gaps or barriers to access
• Lacks evidence-based recommendations or proper citations
• Does not include evaluation plan
