Provide a suggested course of action for the clinic staff. Given the daughter\u2019s age and the situation presented, integrate concepts developed from different psychological content domains to support your suggested course of action. Be certain to use evidence-based psychological concepts and theories to support your arguments. If so, what are they?<br \/>\nIs further information required in order for you to create an ethically sound suggested course of action?<br \/>\nPSY699: Master of Arts in Psychology Capstone<br \/>\nWeek Five Discussion Case File<br \/>\nCase &#8211; Summary<br \/>\nClient: Victoria, 16-year-old HIV positive Latina<br \/>\nConsultation and suggested course of action regarding disclosure of Victoria\u2019s HIV positive<br \/>\nstatus was received 4 years ago. Clinic decision was to honor mother\u2019s request that HIV status not<br \/>\nbe disclosed to her daughter Victoria, who was then 12 years old.<br \/>\nCurrent issue: Victoria is now 16 years old and has a boyfriend. Victoria has explained to her<br \/>\nphysician at the clinic that she and her boyfriend are sexually active and do not always use<br \/>\ncondoms. Victoria is unaware that she is HIV positive because of her mother\u2019s insistence on nondisclosure. The physician has not broken Victoria\u2019s confidentiality about sexual activity, but has<br \/>\nexpressed to her mother, Tina, that it is time for Victoria to know her diagnosis. This is based on<br \/>\nVictoria\u2019s age, the statistics that many girls are sexually active at this age, and that Victoria could<br \/>\nendanger her partner(s). Tina continues to object to disclosure. She states Victoria\u2019s regular<br \/>\nchurch attendance and her belief that Victoria will remain abstinent until marriage as evidence for<br \/>\nnon-disclosure.<br \/>\nThe staff is again wrestling with whether or not they should inform Victoria that she is HIV<br \/>\npositive. In doing so, they would enable her to make informed decisions about exposing her<br \/>\nboyfriend to the sexual transmission of HIV. However, her legal guardian (her mother Tina) does<br \/>\nnot wish for this information to be disclosed to Victoria, who may still be considered a minor.<br \/>\nThe clinic staff is concerned as this situation presents several ethical dilemmas and requires<br \/>\nfurther consultation.<br \/>\nPrevious Case &#8211; Summary (from 4 years ago)<br \/>\nClient 1: Tina, a 36-year-old HIV-positive Latina woman<br \/>\nClient 2: Tina\u2019s daughter, 12-year-old Victoria (also HIV positive)<br \/>\nTina became infected through a former boyfriend who had a history of intravenous drug use.<br \/>\nTina gave birth to an HIV-positive daughter, Victoria. Tina does not want Victoria to know that<br \/>\neither of them has HIV.<br \/>\nVictoria is now 12 years old and has been told by her mother that she takes medications for \u201ca<br \/>\nproblem in her blood.\u201d Recently, Victoria stated that she does not like taking the medication and<br \/>\noccasionally misses doses. The clinic staff has raised the issue of whether Victoria should be told<br \/>\nabout her diagnosis. They\u2019ve warned Tina that in the near future, Victoria will be at an age at<br \/>\nwhich girls often become more interested in boys or sexual behavior. The clinic\u2019s therapist feels<br \/>\nthat if Victoria knew her diagnosis she might be more adherent to her regimen of medications.<br \/>\nHowever, Tina absolutely does not want her daughter to know. Tina believes Victoria is still too<br \/>\nyoung and will be emotionally devastated. Tina believes that it is her responsibility \u2014 and only<br \/>\nher responsibility as a mother \u2014 to \u201cprotect\u201d her child, and that her daughter is \u201cnot ready\u201d to<br \/>\nknow. Tina also believes that Victoria is \u201ca good girl\u201d and will not be sexually active until she is<br \/>\nmarried.<br \/>\nThe clinic\u2019s therapist thinks Tina\u2019s guilt about having transmitted the virus to her daughter is<br \/>\ncausing her to take this stance. Still, the clinic staff is concerned and wants Tina to reconsider.<br \/>\nThis situation presents several ethical dilemmas and requires further consultation.<\/p>\n<p>As a consulting psychologist, I would suggest using the APA Ethical Principles and Code of Conduct as a guide to address the ethical dilemmas presented in the case. Specifically, the principles of Beneficence and Non-maleficence, Fidelity and Responsibility, and Respect for People&#8217;s Rights and Dignity are relevant to this case.<\/p>\n<p>In terms of the suggested course of action for the clinic staff, I recommend informing Victoria of her HIV status in a confidential and sensitive manner. This will allow her to make informed decisions regarding her sexual behavior, including the use of condoms and informing her partner(s) about her status. The staff can also provide education on safe sex practices and the risks of HIV transmission to Victoria and her boyfriend. Additionally, the staff can work with Victoria and her mother to address any emotional concerns or questions that may arise from the disclosure of her status.<\/p>\n<p>Regarding the specific questions posed in the case, the staff should encourage Victoria to inform her mother of her sexual activity, but ultimately, it is her decision whether to do so. Knowledge of Victoria&#8217;s sexual activity may influence her mother&#8217;s stance on disclosure, but this should not be a determining factor in the staff&#8217;s decision. Breaking confidentiality and informing the mother that her daughter is sexually active would violate Victoria&#8217;s right to privacy and confidentiality, and should not be done without her consent, unless there is an imminent risk of harm to herself or others.<\/p>\n<p>The staff should also encourage the mother to inform Victoria of both her and her daughter&#8217;s HIV status, as this information is essential for Victoria to make informed decisions about her health and sexual behavior.
